AHRQ Patient Safety Culture Survey



The Agency for Health Care Administration (AHCA) in Florida has established rule 59A-35.115 for Patient Safety Culture Surveys (PSCS), requiring all hospitals and ambulatory surgical centers to conduct PSCS and report to AHCA biennially. The initial reporting period will run from June 1st to August 31st, 2025. Florida is the first state making the PSCS mandatory. 

Facilities have the option to engage a vendor to collect, compile, and submit the survey data.
